Carle at Work offers a suite of Health Assessment services that provide the crucial foundation for data-driven wellness programming tailored to your workforce’s needs. Assessments empower your employees with insights while providing key metrics for your organization to optimize health and productivity.

Health Risk Appraisals: Questionnaire screening to identify lifestyle risks and stratify employees by health status. Provides baseline metrics for interventions.

Blood Pressure Screenings: Essential biometric tracking of this critical cardiac risk marker. Allows early detection of hypertension.

Cholesterol Screenings: Measures levels of total cholesterol, LDL, HDL and triglycerides – key indicators of heart health.

Functional Movement Screens:  Portable technology accurately captures objective measurements of range of motion and displays a personalized 3D body scan with movement visualizationsThe purpose of the functional movement screens with active and sedentary populations allows our fitness professionals to prescribe corrective exercises that are able to be done deskside, in the field, or literally anywhere. Regular adherence to a corrective exercise prescription will increase mobility and flexibility throughout the body which can improve posture, reduce pain and discomfort, prevent injury, improve performance in situations where physicality is required (e.g. police, fire, waste management) and improve neuromuscular control and movements.

Our wellness portal allows employees to connect to wellness or company resources from anywhere.

The portal allows for easier administration and communication of programming. The portal will be branded and customized to meet your company’s unique needs.

Worker’s Compensation Case Coordination:

  • Case Coordinators are located in all Occupational Medicine locations
  • Coordinators assist with employer notifications of work status after visits regarding work restrictions, obtaining pre-authorization from work comp carriers for follow up testing and treatments, arranging referrals to specialty providers and are the line of communication between all parties involved in a workers compensation case.

Injury/Illness Forms:

  • These forms serve as an immediate source of information and documentation for patients, employers, and adjustors while awaiting progress notes. They include any work restrictions, follow up appointments and/or referrals.
